Affinis Hospice provides a comprehensive approach to the care and well being of patients facing a terminal illness. Individualized care provided by a collaborative team of medical personnel, chaplains and local volunteers, is designed to meet the varied needs of both the patient and their family members.

The Affinis Hospice team provides the following services either at an Affinis qualified center located throughout greater Georgia, or at the patient’s home:

- Physical, occupational therapies and speech-language pathology services
- Bereavement support services
- Pastoral services
- Nursing services available 24 hours/day, 7 days/week
- Home health and Homemaker services
- Medication management services
- Standardized medical equipment

Who is eligible?
Hospice welcomes any patient with a qualifying hospice diagnosis under medicare guidelines.
Who pays for hospice care?
Affinis Hospice is a not-for-profit organization offering comfort and dignity to patients and their families. Hospice care is a covered benefit under many private health insurance plans, and is covered by Medicare and Medicaid. Affinis accepts patients regardless of their ability to pay.
Can the patient continue to use their current attending physician?
Patients may choose any physician to oversee their medical needs. Physicians support of the Hospice program is essential in the care of the patients we serve.
Where is hospice care available?
Hospice care is available in the comforts of their own home as well as in an Affinis qualified center. A local care team will be available to coordinate the patient’s care.
Does the patient have to sign a living will or a DNR (do-not-resuscitate) order to enter hospice care?
No. Patients do not have to sign a living will or a DNR to enter hospice care. At Affinis Hospice, our fundamental philosophy centers around comfort care.
Affinis Hospice can provide patients with all levels of hospice care, these include:
Home care
The patient’s personalized team works together to provide for the needs of the patient within the com-fort of their own home. Our hospice team is on-call 24 hours/day, 7 days/week.
Respite care
For families that may need to take time off from caring for their loved one at home, patients can be trans-ferred to an Affinis Hospice Center for short-term care on an interim basis.
Continuous care
Around-the-clock care provided by our hospice team in a patient’s home during times of elevated discomfort for patient.
In-Patient care
Care provided to the patient in one of Affinis’ qualified centers or a contracted healthcare organization.
